    The information provided below will need to be filled out completely, printed, signed and mailed to the studio with your first month tuition and Family registration fee. Registration is not complete until we receive everything.
    Instructions:
    1. Complete this form.
    2. Print, sign and date.
    3. Submit a copy of this form, your first months payment and the $20 family registration fee to the studio.
    – Tuition is due the 25th of the month prior.
    – $20 Tuition Late Fee
    ​– Return Check fee $30
    – $20 Annual registration fee per family non-refundable
    Tuition Schedule:
    • 45 minute - 1 hour class = $55/month
    • 1.5 hour of classes = $70/month
    • up to 2 hours of classes = $85/month
    • up to 3 hours of classes = $110/month
    • up to 4 hours of classes = $140/month
    • up to 5 hours or more of classes = $165/month
    • 10% sibling discount
    Photography Waiver.
    The studio would like to publish photographs of classes and events on our website, Facebook, etc. We will never, ever, include any personal identification without specific consent.

    Volunteer Opportunities
    The studio always needs additional help with events such as the fundraiser and recital. If you're willing to lend a hand at any of these events, please indicate so here:
    If a student has a cell phone, and would like to be added to a contact list for emergency/important text notices; such as class cancelations, weather delays, etc. Please provide here.
